Heart Failure Educational Modules

The modules are self-contained and written in easy to understand language. Each module provides practical information on a specific topic that is useful to both patients and their families. Each module also includes questions should ask their doctors or nurses. Patients should actively work with their health care providers to achieve the best outcomes.

HFSA Risk Assessment Toolkit

The Heart Failure Society of America (HFSA) wants to assist providers with educating patients about heart failure risk and prevention. The digital materials have been designed to help providers integrate patient information into their practice.

The Health Care Provider & Patient Toolkit has been developed to allow easy and quick access to prevention management tools, to promote the HF guidelines with simple and clear messages and ultimately, to raise awareness on Heart failure risk management.

  • Provide guidance to providers offering clinical preventive care, health promotion, and disease management services on ways to use HF health risk assessment tools followed by evidence-based health improvement programs.
  • Ultimately, reduce health disparities through the use of HF health risk assessment tools and follow-up interventions that are linguistically and culturally tailored and are available to persons with disabilities and other underserved populations.
  • Improve health outcomes by identifying patients’ modifiable health risks and providing follow-up behavior change interventions.

Heart Failure Patient App

The Heart Failure Health Storylines mobile app is created with input from people with heart failure and congestive heart failure (CHF) so that the right health tools are available to patients and caregivers impacted by heart failure. Now it’s easy to record symptoms, vital signs, medications, and more.
