Please Read: All current 2011 HFSA Members have been sent an email with details on how to renew your membership (10/17/11).

Dues for 2012 can be renewed either online or by downloading the 2012 HFSA Membership application.

In order to continue member benefits without interruption, including the Journal of Cardiac Failure, payment must be received by February 1, 2012. Back issues of the Journal will not be available for those who renew membership after April 15, 2012.

Renew On-line

To renew your membership on-line please review and follow the steps below:

1. You will need your user ID:
   Your user ID is your email address on file at the HFSA. If you believe you are a current member and did not receive an email please contact the HFSA at 651.642.1633 or email membership@hfsa.org for assistance.
   
   
2. You will need your password to access your account:
   If you've forgotten your password you may reset it by clicking here to reset your password.
   
   
3. Click on Renew On-line or go to https://memex.hfsa.org/webapps/login.htm to log in with your:
   
   
   • Email address (User ID)
   • Password
   
   
4. Once you are logged in you will see a hyperlink in the upper left corner titled "Member Renewal" that will walk you through renewing on-line.
