Two Ways to Register
For those who pre-register badges may be picked up at the The Peabody Orlando,
Exhibit Hall Convention Level beginning NOON Saturday, September 21st.
Credit card only: American Express, Visa, Mastercard. If paying by credit card,
you must register on-line.
- HFSA Members: After you have logged in, please verify that you are a
member by clicking on "search" after you have entered your first name, last name
and zip code. This will assure that you are charged the membership rate. Failure
to search will result in your being referred to the nonmember rates. If you have
not renewed your membership for 2013, please call the HFSA at 301-718-4800 prior
to registering for the meeting or you will not receive the discounted rate.
- HFSA Nonmembers: After logging in, simply
follow the instructions for registering. Prior to July 30, 2013, nonmember registration rate will include 2013 HFSA
membership dues and a subscription to Journal of Cardiac Failure.
Once registered a confirmation will be sent by to you by e-mail.

If paying by check/money order, remember:
- Registration Forms will not be processed without payment
- Purchase orders are not accepted
- Wire/bank transfers are not accepted
- All requested information must be provided in order for
Registration Forms to be processed.
Registration Cancellation / Refund Policy
If notice of cancellation is received in writing prior to Friday, August 9, 2013, registration fee
will be refunded less a $75 administrative charge. Registration cannot be transferred. No
refunds will be granted after August 10, 2013.
On-site registration will be open Saturday, September 21, NOON - 5:00 PM, The Peabody Orlando,
Exhibit Hall Convention Level.
VISA, MasterCard, and American Express credit cards, checks or money orders (drawn on US banks in US dollars only) accepted.
Checks should be made payable to Heart Failure Society of America.
Fellows, Interns, and Residents registering on-site will need a letter from their training director verifying trainee
status in order to receive the reduced trainee registration rate.
A one-day registration rate is available on-site only. Access to scientific sessions and the exhibits is for the day of purchase only.
