Presenter Disclosure Information & Slide Template
A disclosure slide is required for each oral presentation being given for satellite programs and for the main scientific program. The slide should address all relationships you may have with
commercial entities with an interest in the subject matter of your presentation.
Although a disclosure slide for moderators is not required, moderator disclosure information will
be provided as part of the Disclosure Booklet distributed in the annual meeting bag to all registered attendees.
This slide will be the first slide shown as part of the speaker's presentation.
