HFSA invites applicants to submit their forms online or download the HFSA Membership Application.

Important Note for Membership Applicants

HFSA membership is based on the calendar year of January 1 through December 31. Any HFSA member joining or renewing after August 1, 2016 will have their membership become active on January 1, 2017 and remain active until December 31, 2017. Delivery of JCF subscription will be effective on January 1, 2017.

Physicians – $300

Full Voting Member – Membership dues are on a calendar year basis
(January – December)

MDs, DOs or equivalent who are in good standing in their respective communities, and are actively engaged in heart failure practice or research.

 Non-Physicians -$200

Full Voting Member – Membership dues are on a calendar year basis
(January – December)

All non-physicians who are in good standing in their respective communities, and are actively engaged in heart failure practice or research.

Trainees – $75

Open to individuals enrolled as “full-time” medical students, residents, fellows, undergraduate students, and pre- and post-doctoral trainees

A letter of verification, including the end of training date, from the training program director is required with payment of dues for each year membership is requested.

Documentation MUST be received within 30 days of enrollment/renewal by faxing a letter to 888-213-4417 or your status will be changed to Full Voting Member and your credit card will be charged the difference.
